Creating a multimedia question

To create any new question, you need the 'Create question' permission

Creating a text question

Select Insert > Question. Alternative method

Click the Questions shortcut button located on the toolbar beneath the menus. The question list is displayed. Right click in the white space within the list and select Insert Question Definition from the popup menu.
Enter a code for the new question. Help on creating codes...
Enter a name for the new question. Help on creating names...
Select Multimedia from the Question Type drop down list.
Click OK to create the question.

The appearance of a multimedia question

A multimedia question is displayed on the eForm as a paperclip icon. See image

When the button is clicked, an Open File window enables the user to navigate to and select a file to attach. Once a file has been attached, the icon changes to a magnifying glass. See image
