A comment is a single piece of text that is attached to, and closely associated with, an individual question. This feature is typically used to make clinically significant comments on a question whereas notes are generally used as a personal reminder to the creator. Check the procedure with your system administrator as they may wish you to use comments and notes in a particular way.
Only one comment can be added to a question but it can be edited at any time.
A comment can only be added to a question once the question has been answered.
A comment cannot be added to data that has been frozen or locked.
A record of the addition, changes to and deletion of all comments is kept in the audit trail for the associated question. This is how comments differ from notes, which can be kept private and are stored separately from the question.
