eForm dates and labels

You can choose to display a date and/or a label under an eForm icon in the data entry schedule. See image

 

You must set up eForm dates and labels individually for each eForm. There is no way to switch them on/off for an entire study.

Setting up an eForm date

 To create an eForm date you must set up a special question on the eForm, as follows:

  1. Create a date question. You can leave it to be answered directly by the data entry user or you can enter a derivation to use the response from another question. If you leave it as user-enterable, the data entry user will not be allowed to save the eForm until a valid date has been entered.

  2. Place the date question on the eForm and adjust the caption if necessary.

  3. Edit the eForm definition and click 'More' to open the full window.

  4. Locate the 'Date' drop down list at the top. This list contains all date questions on this eForm.

  5. Use the list to select the question you have just created. See image

  6. Click OK to save the eForm definition.

 

Because the new date question has been assigned as the special eForm date question, it will not appear on the eForm in Data Entry like other questions. Instead, it will appear at the top right in the header of the eForm, with the caption you specified. See image

When the question is answered, the date value will be displayed in the header of the eForm and also on the schedule underneath the eForm icon. See image

Setting up an eForm label

An eForm label can either be taken from the response value for a nominated question, or it can be an Arezzo expression.

To create an eForm label using a response value:

  1. Edit the eForm definition and click 'More' to open the full window.

  2. Locate the 'Label' drop down list at the top. This list contains all questions of all types on this eForm.

  3. Use the list to select the question you wish to use and click OK to save the eForm definition.

 

When the question is answered during data entry, the response value will be displayed on the schedule underneath the eForm icon.  See image

The label is not automatically displayed on the eForm itself.

To create an eForm label using an Arezzo expression:

  1. Edit the eForm definition and click 'More' to open the full window.

  2. Locate the 'Label' field at the top. Click the Expression Editor button next to it.

  3. Enter the required expression and click OK to close the editor, then click OK to save the eForm definition.

 

The expression will be evaluated during data entry, and the value will be displayed on the schedule underneath the eForm icon.

 

There is no length restriction in the display of eForm labels. So if you create a label that uses a response from a question which allows a long value, the entire value will display under the eForm icon. This will expand the width of the column which may cause the schedule to look untidy and become difficult to work with.

 

If the information referenced in an eForm label or the expression itself is changed or deleted, the label will be automatically updated.

Visit dates and labels

If you set an eForm to be a visit eForm, its eForm date and /or label will also be used as the visit date/label.
