Creating an eForm

To create a new eForm, you need the 'Create eForm' permission

A well-designed eForm looks attractive and is easy to use. Users can navigate around the eForm, enter and retrieve data more quickly and accurately.

To create a new eForm, you can do one of the following.

Then:

  1. Enter a code for the new eForm. Help on creating codes...

  2. Click OK to create the new eForm or Cancel to abort.

  3. The new eForm will be displayed in the question list. If there is no plus sign to the left of the eForm name, that means that no questions have been placed onto the eForm so far.
If you click the 'eForms' shortcut button, you will see that a tab bearing the name of the new eForm has also been added to the horizontal row of eForm tabs across the top of the screen. The new blank eForm will be displayed underneath the tab.

Duplicating and copying eForms

We recommend that once you have designed a basic eForm, you duplicate it by creating as many copies of it as there are eForms in your study.

Note: This is not possible within the Library Management module.

It is also possible to copy whole eForms into the current study from the library and from other studies.

The Arezzo Terms report in the Study Data Diagnostic Utility module can be used to list the location of all eForms in a study that contain an Arezzo term.

If it has been configured to appear as a link on the home page, the Study List report gives details about eForms used in a study in its eForm Summary sub report.
